Evaluation of a telephone call service for ambulatory surgery patients in taiwan.

J Nurs Care Qual. 2007 Jul-Sep;22(3):286-8

Authors: Chen CH, Ma HJ, Yen M, Li SF, Liu LC

The purpose of this study was to evaluate the outcomes of a telephone call service (TCS) for ambulatory surgery patients. Three outcomes were evaluated: patient satisfaction, patients' perceptions of the value of a TCS intervention, and nurses' perceptions of difficulties in implementing it. The results indicated that patients were satisfied with the TCS intervention and were positive about its value. Nurses indicated that the main difficulties in implementing the TCS were their lack of knowledge in answering patients' questions and time limitations.

J Nurs Care Qual. 2007 July/September;22(3):279-285

Authors: Censullo JL, Mokracek M, Newmark M

The 10-year aftermath of Shortell and colleagues' landmark intensive care unit study has resulted in healthcare institutions increasingly strategizing themselves into disease-specific niches. Specialization has evolved to the point at which The Joint Commission will certify exceptional disease programs. One such certification has been developed for stroke. The processes essential to attain stroke certification are detailed, complex, and outlined in this article.

Inpatient satisfaction with nursing care and factors influencing satisfaction in a teaching hospital in china.

J Nurs Care Qual. 2007 Jul-Sep;22(3):266-71

Authors: Liu Y, Wang G

This study examined inpatient satisfaction with nursing care in a teaching hospital in China. Patient satisfaction with nursing care was assessed by a self-designed questionnaire administered to 320 inpatients selected by a convenience sampling method. The patients had a relatively high level of satisfaction with nursing care. Patients' age, educational background, occupation, methods of payment, and hospital wards were main factors influencing their satisfaction with nursing care. The study provides a new tool for measuring patient satisfaction with nursing care in China.

Evaluating an Educational Approach to Improve Pain Assessment in Hospitalized Patients.

J Nurs Care Qual. 2007 July/September;22(3):260-265

Authors: Michaels TK, Hubbartt E, Carroll SA, Hudson-Barr D

Pain assessment is a multifaceted process. A common assumption is that all nurses have the same baseline knowledge about pain, a potentially erroneous assumption that influences clinical practice. Nurses have varied experiences in education and pain management. This article describes a research project conducted by the hospital's clinical nurse specialist group to evaluate the effects of a nursing education program on pain assessment and pain management of hospitalized patients in an 841-bed academic medical center.

Association of Age and Quality of Life Following Phase II Cardiac Rehabilitation.

J Nurs Care Qual. 2007 Jul-Sep;22(3):255-9

Authors: Kardis P, Sherman M, Barnett SD

The benefits of cardiac rehabilitation following a major cardiac event are without question. Less well known, however, is the association of age with improved quality of life following rehabilitation. Subjects (n = 300) completed a 3-month phase II cardiac rehabilitation program following either a coronary artery bypass graft or a valve procedure. There were similar gains in health-related quality of life across all age groups; our results suggest that there needs to be a greater focus on emotional needs among our younger patients.

A cardiac surgical progressive care unit implemented the ABC's of Cardiovascular Risk Reduction Care Bundle to determine whether the use of a packaged approach to medication prescription and lifestyle counseling would improve adherence to secondary risk-reduction guidelines in postcoronary artery bypass graft patients. A pilot study was carried out to assess changes in adherence to guideline recommendations post-Care Bundle implementation. Findings support using a systematic strategy to improve guideline adherence in this population.

J Nurs Care Qual. 2007 Jul-Sep;22(3):239-46

Authors: Darmody J, Pawlak R, Hook M, Semeniuk Y, Westphal J, Murray ME, Henriques J

Substitution of hospital staff performing concurrent utilization review (CUR) was evaluated using a production process framework. There were no differences in the number of reimbursement denials or denied days among 4 job classifications of hospital staff performing CUR, indicating that educational preparation of staff did not affect outcomes. The implications are that hospitals could substitute assistive staff in place of registered nurses to complete the CUR function, potentially increasing the availability of professional nurses.

Optimizing nursing care by integrating theory-driven evidence-based practice.

J Nurs Care Qual. 2007 Jul-Sep;22(3):234-8

Authors: Britt Pipe T

An emerging challenge for nursing leadership is how to convey the importance of both evidence-based practice (EBP) and theory-driven care in ensuring patient safety and optimizing outcomes. This article describes a specific example of a leadership strategy based on Rosswurm and Larrabee's model for change to EBP, which was effective in aligning the processes of EBP and theory-driven care.

There is increasing effort in promoting evidence-based practice (EBP) that supports the best possible care to patients and families. This article describes essential concepts for developing an environment of EBP and its implementation at a large pediatric hospital. Essential components for creating an EBP environment include vision, engagement, integration, and evaluation. An institutional initiative to decrease procedure-related pain demonstrates how EBP is used to improve clinical care.

J Nurs Care Qual. 2007 July/September;22(3):213-221

Authors: Grant MJ, Larsen GY

Adverse event reporting is a key element for improving patient safety. This study describes a new voluntary, anonymous reporting system that facilitates reporting of near-miss and patient harm events and an assessment of patient harm by the bedside care provider in a pediatric intensive care unit. The results demonstrated the effectiveness of the Patient Safety Report as a method to capture near-miss and patient harm events.

Authors: Pelander T, Leino-Kilpi H, Katajisto J

This study assessed the quality of pediatric nursing care as perceived by children (N = 388) aged 7 to 11 in Finland. The children rated as excellent the nursing environment, the nurses' humanity and trustworthiness, and caring and communication. They were most critical about the nurses' entertainment activities such as playing with them. Children's age and type of admission were both related to the amount of information they received. The instrument used in the study needs to be developed and tested further.

A plan for achieving significant improvement in patient safety.

J Nurs Care Qual. 2007 Apr-Jun;22(2):164-71

Authors: Johnson K, Maultsby CC

Improvement in systems that ensure safety in the provision of care is a high priority to hospital administrators, clinicians, and patients. Research to determine the approaches and methods that will result in the most significant patient safety improvements is underway but more is needed. This article describes the process for improving patient safety adopted at one hospital. Results of these efforts demonstrate significant improvement in staff understanding of patient safety measures. Staff survey results are supported by improvement in clinical indicators. Recommendations for future action and implications for other hospitals are discussed.
